Effective January 1, 2020, OHIP will no longer cover any portion of your medical costs while travelling outside of Canada.

Change Beneficiary

A Member may change their beneficiary at any time. The Member is required to complete the Pension Plan Appointment or Change of Beneficiary Card. This card is available from your employer, or administrative agent.

The following information must be identified on all change of beneficiary cards and returned to the administrative agent for processing.

  • Employer's name
  • Member information
    • Name
    • Social insurance number
    • Date of Birth
    • Gender
  • Beneficiary's information
    • Name
    • Social insurance number
    • Date of Birth
    • Relationship to Member
  • Current date, with your signature (front and back) along with that of a Witness (front)
